Summarise a Medical Appointment Into a Note You Can Actually Use
You left the appointment and by the time you reached the car park you had forgotten two things the doctor said. This turns what you do remember into a clear note: what was decided, what you need to do, and what you did not get to ask. Useful for the next appointment and for anyone else in your household who needs to know.
Write down everything you remember from a recent medical appointment: what you described, what the doctor or nurse said, what was prescribed or recommended, any follow-up actions, and any test results or referrals mentioned. Then summarise it into a usable note covering: the key diagnosis or assessment, the agreed treatment or next steps, any lifestyle changes recommended, dates for follow-up or results, and any question you did not get to ask. Keep the note under 200 words. Flag anything you are uncertain about that is worth clarifying at the next appointment.
